The Independent Review Panel cannot consider your case until you have completed the Equitable Life Payment Scheme complaints process.
Once you have completed the complaints process and received a Independent Review Panel Referral number, you will need to complete the Review Request form enclosed with your letter to have your case reviewed by the panel. When you have done this you may write to the Panel at:
